We require guardian details to ensure the safety and well-being of our younger members. This information helps us contact a responsible party in case of emergencies and ensures compliance with legal requirements for minors participating in fitness activities.
I, the undersigned guardian, give permission for my teen (under 16) to use the gym facilities. I understand that I am responsible for supervising my teen during fitness sessions. If I choose not to supervise, I accept full responsibility for their actions and safety. I acknowledge that the gym is not liable for any incidents, injuries, or issues that may arise from my teen's use of the facilities without my supervision.
